Three social workers have been suspended for ignoring warnings about a father who later attacked and brain damaged his baby daughter.

Zak Whitlock, 21, was allowed to return to the family home in Wigston despite having a history of mental illness, violence, drug abuse and criminality.

Leicestershire County Council's children's services director, Gareth Williams, said: "At the heart of this case was a serious error of professional judgement made by social care professionals."

A serious case review into the scandal unearthed a series of blunders by childcare professionals. Its results were released at a press conference today.

Mr Williams said he "conceded" that a warning from a trainee probation officer four months before the child was born that Whitlock was a risk had not been acted on.

Documents detailing the risk Whitlock posed were also lost or inadvertently ignored, while it was also found that social workers were too trusting with Whitlock and did not challenge his account of events.

Two frontline social workers and their manager are currently suspended on full pay. Meanwhile four senior social services directors have also moved on and been replaced.

Mr Williams said: "I would like to say straight away how sorry I am that this child suffered such tragic injuries.

"This is a very serious case and in the light of that we have suspended three members of staff and rigorous disciplinary procedures are ongoing."

    “What this newspaper has called forced adoptions actually accounts for most adoptions, many of which happen against the wishes of parents, who are known to be a risk to the child. There are very few relinquished babies in the 21st century. Forced is probably not a good word for this, rescued is probably better?”

    “I am not the same K as commented earlier. Why was this person allowed to return to the family home by the social services? Has he now been arrested and charged with inflicting these terrible injuries on this innocent child?”

    “"A serious error of professional misjudgement made by social care professionals" and "a series of blunders made by childcare professionals" surely is sufficient grounds for INSTANT DISMISSAL.

    It certainly would be in many areas outside the public sector.

    Just how many cases will it take for appropriate action to be taken?”

    “So the social workers, in this case, have been suspended ON FULL PAY and some managers have moved on!
    Meanwhile the flaws in the system which enabled (some would say aided) the IDENTIFIED at risk child, in this case, remain in place and have not been recognised or rectified.
    The reality is that many IDENTIFIED being abused at risk children, up and down this country, are being failed to be protected from being further mind and body twisted by the very system which is only in existance to help them.
    Enough. There is something seriously wrong with THE NATIONAL child protection policy and with child protection as it is in practice.
    Which brave MINISTER will take the bull by the horns and lead a government lead effort which will recognise that there is an URGENT need to redesign child protection in practice, from root go branch, so that it can work for the children and so that it is 'workable' for all those who try so hard to protect them?
    Those of us, on the frontline, who have had to report 'the seen' abuse of children and who have tried to use the system to protect them must have our EXPERIENCE EXAMINED and the findings used as part of any effort which must surely going to be made NOW to put the children BACK into frontline child protection, in practice.
    The IDENTIFIED being abused at risk children are in dire need of a REAL COMMITTED MINISTER FOR CHILDREN who will speak out and act for them!”

    “I am a SOCIAL WORKER and would like to know why people think this is a highly paid job...please!!!....people enter into this profession to protect the vulnerable both adults and CHILDREN... PEOPLE Lie yes, it is our job to investigate but our hands are tied on MANY occassions and something needs to be done about social workers having extra powers to go into homes or other places to protect the vulnerable first and then look at the rights of parent/carers??.”

    “they should be sacked, not any other outcome,they are people who deal with children well beings, then they let children down,the director should be accountable also,if they think they are untouchable, when dealing with people,there should be a law to deal with them and not let them hide behind a closed door.also it stood be made public on all reports, and notto let somebody go out and make a weak SORRY to the people of leicestershire,as who pays there large salary and high pensions, we the people of leicestershire, so we must have a say in it all”

    “Enough. Another IDENTIFIED at risk child has be enabled, some would say aided, to die because of a NATIONAL child protection in practice situation which is NOT working for MOST of the very children that 'the system' is in existance for!
    Which brave MINISTER is going to take the bull by the horns and EXAMINE what is going wrong for so many of the IDENTIFIED most vulnerable children, in this country?
    Those of us who have had NO CHOICE but to report that children, who we know well, are being abused have much to say regarding how we and our concerns raised for the children are being responded to, accounted for, recorded, documented and most of all followed-up.
    We have many good ideas as to how the situation and the outcomes for many children can and could be improved.
    Our EXPERIENCE of child protection, in practice, must be EXAMINED and the findings used to build a much better way forward.
    For The Sake of The Children!”
